New or Transferring in to Holyoke Pediatrics

Prior to booking your first appointment at Holyoke Pediatrics, we must have your medical records on file.

To transfer medical records to our office you should complete one of the following:

• Sign an Authorization for Release of Medical Records form in our office or mail/fax/email the completed form to the Holyoke office
  
  • Mail to: Holyoke Pediatric Associates, LLP 150 Lower Westfield Road, Holyoke MA 01040 Attention Medical Records.
  • Fax to: 413-536-1087
  • Email to: [email protected]
• Sign an Authorization for Release of Medical Records form at your previous primary care office
• If you already have medical records including immunization history on hand, they must be sent to the Holyoke office

You must have your insurance information available for verification prior to booking. We accept most insurances.  Please call your insurance company to verify that we are in network with them.  If you need additional assistance, please contact our billing department 413-536-2393 option 8.  If a Holyoke Pediatrics provider is not listed as the primary care provider on your insurance policy, please call the member services number located on the back of your insurance card and choose one of our providers prior to booking. All information must be verified with an HPA Customer Associate before booking any appointments. Be sure to bring any and all insurance cards along with your copayments. Copayments are due at the time of service. If copayment is not paid at time of service a $15 billing fee is incurred.

Newborns

Expecting or adopting?  We love meeting expectant parents at Holyoke Pediatrics.  Having a baby is one of the most exciting times in your life and you may have many questions.  Here at Holyoke Pediatrics we are committed to keeping your baby healthy and providing you with the tools needed to parent with confidence.  

When you deliver your baby at the hospital or birthing center, please be sure to notify the hospital staff that you have chosen Holyoke Pediatric Associates as your baby's pediatrician. The hospital’s staff pediatrician will take care of your baby while in the hospital. On the day of your baby’s discharge from the hospital, please call our office to register your baby and schedule a Newborn Visit.  This visit should take place within 1-2 days of discharge from the hospital. Be sure to also contact your employer and/or your insurance company to have your new baby added onto your health insurance policy.
