Medical Records Department

The Medical Records Department is open Monday through Friday, from 8:00 am to 5:00 pm and can be reached by calling
(413) 536-2323 for the Holyoke office and (413) 532-0300 for the South Hadley office.

If you need a copy of your child's medical record, you must come into the office first to sign a Release of Information form. There is a copying fee for each medical record copy requested. There is no charge for medical records transferred to another doctor, clinic, or hospital.

School and camp form completion and immunization records requests are also processed in the department. There is no charge for completing forms and paperwork.

While we endeavor to process requests as soon as possible, please allow 5 business days.

 

Prescription Refill Policy

To obtain a refill on your child's medication, please call our office Monday - Friday between 8:00 am and 4:30 pm, at least 48 hours in advance. Please have the following information readily available:

- Patient's Name

- Date of Birth

- Name of Medication

- Strength and Dose of Medication

- Doctor's Name

- Pharmacy Name and Telephone Number

- A telephone number where you can reached if there any questions.

 

Referral Policy

The Referral Department is open Monday - Friday between 8:00 am and 4:30 pm and may be reached by calling (413) 536-2393 ext. 152 to reach the Holyoke office and (413) 532-0300 to reach the South Hadley office.

Please call at least 48 - 72 hours in advance with your referral request. It is the responsibility of the insured to monitor the necessity of obtaining a referral and how many specialist visits are covered. Please contact your insurance company to find out whether a referral is necessary.

 

 

On-Site Lab

Hours of Operation:

The lab at the Holyoke office is open Monday - Friday from 8:00 am to
5:00 pm for blood draws and specimen drop offs. The lab is closed week-ends and holidays.

Scheduling an Appointment:

If you are instructed by your provider to return on another day for lab work or to drop off specimens (urine, stool, etc.) we ask you to call ahead of time so an appointment can be made in the lab schedule (413) 536-2393. This will give us an opportunity to ensure your medical record is in order and expedite your visit.

If you prefer to use another lab for blood work or specimen drop offs please make your provider aware of this so they can give you the appropriate paperwork.

Questions:

If you have questions regarding the collection of lab specimens we can be reached by calling the main number at the Holyoke office (413) 536-2393 and then asking for the lab.

If you have questions regarding the results of your lab test you should not call the lab as lab personnel cannot give out lab results. As lab results become available they are given to the ordering provider.

Blood Draws:

The lab staff at HPA have many years of combined phlebotomy experience and are sensitive to the issue of drawing blood on children of all ages. We always strive to make the blood drawing experience a positive one through encouragement and rewards. Again we ask you to call ahead of time so an appointment can be made in the lab schedule (413) 536-2393.

We have the capability of performing many tests on site. However if we can not perform a test on site we send all specimens to Baystate Reference Lab for processing.

We can only draw blood on patients whose primary care provider is at HPA. If your primary care provider refers you to a specialist and the specialist orders blood work we can in turn draw that lab work also.
